ABSTRACT

Objective To explore the clinicopathelogical features of resectable carcinoma of the head of (pancreas).Methods A retrospcctive analysis was made on the clinicopathological data of 102 patients with cancer of the head of pancreas, who had received pancreatoduodenectomy from Nanfang Hospital in January 1990 to January 2003. Results The incidence rate of the peri-pancreatic tissue infiltration was 74.5%,the infiltration rate of retroperitonealfat was 27.5% and the incidence rate of peri-pancreatic lymph-node (metastasis) was 71.6%. Metastasis rate of 21.1% was seen in the abdominal aorta lymph nodes. (Conclusions) Surgically resectable carcinoma of the pancreatic head is not equivalent to early cancer. The surgical area of radical resection of cancer of the pancreatic head should at least include pancreatoduodenectomy and clearance of regional soft tissue and lymph nodes. It may be more reasonable if the abdominal aorta lymph nodes were assigned to the first station of lymph drainage of carcinoma of head of the pancreas.

ABSTRACT

Objective To study the surgical treatment of major blood vessels invaded by tumors in order to raise the resection rate and postoperative survival rate. Methods Clinical data were analyzed on 26 patients undergoing (resection) of tumors along with reconstruction of major blood vessels from October 1998 to February 2004. Results Tweety-three cases were followed up for 2~65 months with median 42.8 months. Doppler ultrosonography and CTA examination were performed respectively during the follow-up process.In 5 patients with carcinoma of the head of pancreas who had inferior vein cava replaced and in 1 patient with cholangiocarcinoma who had portal vein replaced,3 were complicated with carcinoma embolism formation at 3,12 and 15 months respectively after surgery,(postoperative) graft patency was 50%;all of the 6 patients had recurrence of the tumor at 3,6,8,12,24 and 31 months respectively after surgery.Seven patients with tumor in pelvic cavity or in posterior space of peritoneum were followed up for about 2 years,1 of them had tumor recurrence ,but the graft kept patent,2 patients with sarcoma in the limbs were followed up for 1 year without tumor recurrence or vessel obstruction.Eleven patients with tumors in the neck were followed up for 3 months to 5 years without tumor recurrence or vessel obstruction. Conclusions (Combination) of resection of neoplasm and reconstruction of major blood vessels is safe,effective and practical method for patients with major wessels invaded by tumor.

ABSTRACT

Objective To explore the expression and clinical significance of EphA2 and E-cadherin in (pancreatic) cancer tissue and cells.Methods The expression of EphA2 and E-cadherin in 56 pancreatic(carcinomas) and 23 adjacent noncancer tissues were studied by immunohistochemistry,and their relationship to clinicopathological characteristics were analyzed.RT-PCR was performed to explore the expression of EphA2,BXPC-3,PC-3 and PANC-1 in pancreatic carcinoma cell lines.Results In pancreatic carcinomas tissuse showed increased EphA2 expression and reduced E-cad expression which compared with adjacent noncancer tissues.The expression level of EphA2 had a significant positive relationship with tumor differentiation degree,lymphatic metastasis and clinical stage.However,the expression level of E-cadherin had negative relationship with both the tumor clinical stage and lymphatic invasion.Furthermore,a significant negative relationship(between) the expression of EphA2 and E-cadherin was observed.The expressions of EphA2 were higher in high-invasive cell lines PXPC-3 and Panc-1 than in low-invasive cell line PC-3.Conclusions The(expression) and/or abnormal function of EphA2 and E-cadherin may together be involved in the development and progression of pancreatic cancer;the combined measurement of these two proteins may be useful for(determination) of metastatic potency of pancreatic carcinoma.

ABSTRACT

Objective To investigate the anti-migratory and anti-invasive effect of somatostatin receptor type 2(SSTR2) gene transfection mediated by adenovirus in human pancreatic carcinoma cell and the mechanisms involved in this effect.Methods The full length human SSTR2 cDNA was introduced into pancreatic cancer cell line BXPC-3 by adenovirus-mediated transfection,and stable expression of RNA and protein of SSTR2 were detected by RT-PCR and Westen-blot.The Matrigel coated Transwell was used to detect the migratory and invasive ability of SSTR2expressing cells,Adv-GFP control cells and mock control cells.Furthermore,the expressions of matrix metalloproteinase-2(MMP-2) and tissue inhibitor of metalloproteinase-2(TIMP-2) were detected by RT-PCR method in these cells.Results The stable expression of SSTR2 was detected in BXPC-3 cells transfected by Adv-GFP-SSTR2.A dramatic decrease of BXPC-3 expressing SSTR2 cell(migrated) through a Matrigel-coated filter was observed,as compared with Adv-GFP control cells and mock control cells(P

Objective To study the effect of intratumor angiogenesis and vascular growth factor(VEGF), basic fibroblase growth factor(bFGF), Platelet-derived growth factor (PDGF), and their receptors(flt-1 bFGFR.PDGFR) on the invasion, metaslasis of pancreatic carcimoma(PC) and the relationship between the expressions of three kinds of angiogenic factors, their receptors and microvessel count(MVC). Methods Tis- sue sections of 51 PC and 32 acute or chronic pancreatitis were examined by in situ hybridization for the expression of VEGF bFGF.PDGF, and by immunohistochemistry for the expression of the three kinds of angiogenic factor receptors and MVC.
